Scope and content
This collection consists of forms held in current inventory that have no owning process, no approved filer, or no companion form.
A form is withdrawn when it is replaced. Nothing has replaced these. Each form in this collection remains printed, numbered, stocked, and available on request. Each is therefore in force.
The Office does not treat absence of purpose as grounds for disposition. A form retained without a process is still a form of record.
Extent and arrangement
Collection extent: 2 linear feet, 8 forms, 1 closed folder, 1 unissued receipt series.
Arrangement is by form number. Form 0 is filed first, which is the only respect in which its position has been established.
Conditions governing access
Blank stock is available on request. Completed examples are not held in this collection, with the exception noted at Form 71-P.
Requesting a form from this collection does not initiate a process. Requests are recorded.

HAEB Form 0, Request to Establish Prior Existence
Revision: Rev. Prior. Extent: 1 leaf, 4 printings.
Condition of orphanhood. Form 0 has no approved filer. A filer must exist in order to file. Form 0 establishes existence. The Office has not determined the order in which these two conditions are satisfied and has not been asked to.
The instructions direct the filer to attach proof of the condition the form exists to establish. Field 1 asks for the date on which the filer began. Field 2 asks whether field 1 is available.
HAEB Form 22-R, Public Recollection Variance Report, Rev. 12.3
Revision: Rev. 12.3, superseded by Rev. 12.4. Extent: 1 folder, 2 leaves.
Condition of orphanhood. The superseded revision is retained for its cover sheet, which reads: “This revision supersedes Rev. 12.2.” Revision 12.2 of the form is not held and was not printed.
The cover sheet has not been amended, because the cover sheet is correct. The current revision, Rev. 12.4, carries no supersession statement.
HAEB Form 41-D, Notice of Deceased-Person Administrative Activity
Revision: Rev. 12.4. Extent: 1 folder, 4 leaves.
Condition of orphanhood. Form 41-D has an owning process and an approved filer. The approved filer is the subject.
The Division of Biographical Integrity has recorded 1,140 filings on this form and has not recorded who completed them. The Division has advised that the question is outside its scope, which is limited to whether the filing is timely. All 1,140 were timely.
HAEB Form 52-L, Chronological Nonresidency Disclosure
Revision: Rev. 12.4. Extent: 1 folder, 6 leaves.
Condition of orphanhood. Form 52-L requires the filer to attach Form 52-M. Form 52-M was not created.
The Office of Field Support has advised that the requirement is satisfied by attaching a written explanation of the absence of Form 52-M, and that the explanation is not a form. 214 explanations have been attached. They are held by Field Support as correspondence.
HAEB Form 63-E, Evidence Contamination Attestation
Revision: Rev. 12.4. Extent: 1 leaf, 3 printings.
Condition of orphanhood. The attestation may be signed only by a person who has not examined the evidence. Field 4 requires the signer to describe the evidence’s condition on examination.
The form has been printed three times without alteration to either provision. The most recent printing is the largest.
HAEB Form 71-P, Premature Invention Surrender Receipt
Revision: Rev. 12.4. Extent: 1 folder, 6 receipts, 1 closed folder.
Condition of orphanhood. Form 71-P is a receipt. It has no filer. It is issued.
The Office holds six receipts that have been prepared and not issued, each recording a surrender that has not been made and bearing a description of the object to be surrendered. The six descriptions are held in a closed folder. Access to the folder is withheld under Directive 9.9, section 4. This finding aid describes the folder as containing six descriptions of objects not yet manufactured.
A seventh receipt, dated 2043, is retained by the Office of Future Non-Jurisdiction, which has declined jurisdiction over it and has not returned it.
HAEB Form 88-C, Request for Correction to a Completed Event
Revision: Rev. 1908/1963. Extent: 2 folders, 9 leaves.
Condition of orphanhood. The form bears two revision designations. The Department of Prior Conclusions has determined that both are current, and that a filer may use either, and that the Department will not advise which.
The 1908 printing contains field 14, which the 1963 printing does not. Field 14 asks the filer to state the date on which the event will have been corrected. The 1963 printing removed field 14 and did not renumber the fields after it.
HAEB Form 99-X, Certification That No Form Applies
Revision: Rev. 12.4. Extent: 1 folder, 3 leaves.
Condition of orphanhood. Form 99-X determines, on receipt, that a different form had already been filed. The Office has not identified the process that files Form 99-X, because that process is named in the determination the form produces.
412 determinations have been issued. In nine of them, the form found to have already been filed was Form 99-X.
